As well as proving to be an effective form of treatment for ischemic heart disease, Percutaneous Transluminal Coronary Angioplasty has provided valuable insight into the way the heart behaves when subjected to ischemic insults. Since the publication of Coronary Angioplasty: A Controlled Model for Ischemia in 1986 (DICM 58) there has been increasing interest in this field with considerable refinement of the original techniques as well as the development of new techniques such as intracoronary Doppler, radioisotope metabolism, and measurement of coronary sinus pH.
As pointed out by Spencer B. King III in his Foreword to the book, the use of all the 'eyes' available to the investigator, 'will advance our understanding and quicken the day when ischemia can be reliably controlled'. Attention has now turned to cardioprotective mechanisms (pharmacological or mechanical) in an attempt to reduce ischemia and prevent myocardial damage. As a result it is likely that in the future more complex and lengthy interventions will be performed with a greater degree of safety.
The second part of the book is devoted to acute ischemic syndromes as the boundary between episodes of brief transient ischemia and these ischemic syndromes is becoming less distinct. The technique of PTCA has been increasingly applied to this area and has unraveled important information on the reversibility of the ischemic or stunned myocardium.
