About the authors
Harry P.A. van de Water (right) is a senior researcher at TNO Prevention and
Health. After taking his M.D. from Leiden University he worked for some years
in clinical medicine and for the Leiden Municipal Health Service. Then he was
trained as a public health specialist and epidemiologist, became a full-time
researcher at TNO, and received a Ph.D. from Erasmus University in Rotterdam
on the scientific and organizational bases of public health. His research
activities cover public health epidemiology and the development, evaluation and
organization of preventive health care. Over a hundred publications have
appeared from his hands. He is actively involved in the development of
integrated indicators of population health and belongs to the leading members
of the International Network on Health Expectancy and the Disability Process
(REVES) and its European compeer Euro-REVES.

About the authors

researcher at TNO, and received a Ph.D. from Erasmus University in Rotterdam

Loes M. van Herten (left) is trained in epidemiology and biological health
sciences at the University of Limburg in Maastricht and also holds a M.Sc.
degree in environmental studies from Utrecht University. She has invested the
biological effects of radiofrequency radiation for the Dutch Health Counsel.
At the World Health Organization's headquarters in Geneva, she worked on the
evaluation of the World No-Tobacco Day 1993 for the Tobacco or Health
Programme, and made a comparative analysis of world cigarette consumption.
Since 1995 she has been working as an epidemiologist at TNO Prevention and
Health. Here she started with the trend-analysis the results of which are
presented in the Appendix of this report. Other fields of interest include health
expectancy indicators, and monitoring of health status and health determinants
of adolescents and young adults.
About TNO Prevention and Health
The key mission of this research institute - located in Leiden, the Netherlands,
part of the Netherlands Organization for Applied Scientific Research (TNO)
- is to contribute to the enhancement of health related quality of life. With an
emphasis on prevention, research focuses on the improvement of both health
and health care at all stages of life; from conception through childhood and
working life to old age. The research divisions of TNO Prevention and Health
cover biomedical as well as medical technology and socio-medical issues. The
study presented here has been carried out in the multidisciplinary Public Health
and Prevention division of the institute, whose expertise in disease prevention,
health promotion, health care systems and epidemiology is made available to
both national and international clients within research projects and consultancy
