In 16 expert chapters, this volume considers and compares the different methods to clinically evaluate coronary perfusion in man. Comprehensive coverage is given to all currently available methods, and chapters stress their relative strengths and limitations and examine their potential value for better understanding coronary pathophysiology and their possible applications in clinical practice, especially for assessing interventions such as PTCA. Highlights of the book regard important concepts on the regulation of the coronary circulation and its adaptation to types of stress, and the theoretical background and practical application of coronary vascular reserve. Based on an international workshop, the text considers in detail the thermodilution method, nuclear techniques, and newly developed techniques. The first group of chapters describes the use of the thermodilution method for assessing coronary flow reserve in various clinical conditions, and for exploring the effects of drugs, neuropeptides, histamine and leukotrienes, and physiological maneuvers on coronary circulation. Several papers then report in detail the advantages, limitations and possible applications to man of nuclear techniques including thallium-201 imaging, the microsphere technique, and positron emission tomography. Subsequent chapters focus on newly developed techniques such as coronary videodensitometry, intracoronary Doppler and contrast echocardiography. These techniques are thoroughly discussed both from the methodological point of view and that of their clinical applications. The volume concludes with a round table where the authors critically review, and define the state of the art and the potential clinical usefulness of the methods described. Individual papers are accompanied by discussions by the experts present of crucial and controversial issues which arose during the workshop. This book will be indispensable reading for all those Interested in an up-to-date and authoritative treatise on the methodology, capabilities and applications of all methods available today for evaluating coronary blood flow in man.
